2. In case of more than one application uses a separate application form along with the original deposit slip.

3. Application must reach the NCBMS office latest by the last date of submission of the application form.

4. NCBMS will not be responsible for late receiving of applications through courier / Pakistan post etc.

5. Attach your recent photograph, CNIC copy, and an original bank deposit slip with this application form.

6. Without a signature & Thumb impression, your application form will not be entertained.

7. Without a photograph your application form will not be entertained.

8. In-complete form will not be entertained. (All the fields are mandatory/Required).

9. By hand submission of the Application form is not allowed.

10. Mobile phones are not allowed on test center premises.

11. Please visit the NCBMS website according to the test schedule to check your status.
