• Application Fee (Service Charges) is nonrefundable/nontransferable. A Bank Online Deposit of Rs. 80/- must be attached to the application form.

• In case of more than one application use a separate application form along with the original deposit slip.

• Application must reach the OTS office latest by the last date of submission of the application form.

• OTS will not be responsible for late receiving of applications through courier / Pakistan post etc.

• Attach your recent photograph (Latest by 6 Months), CNIC copy, and an original bank deposit slip with this application form.

• Without a Signature & Thumb impression, your application form will not be entertained.

• Without a photograph your application form will not be entertained.

• In-complete forms will not be entertained. (All the fields are mandatory / Required)

• In Person/By hand submission of the Application form is not allowed.

• Mobile phones or other electronic gadgets are not allowed on test center premises.

• Please visit the OTS website according to the test schedule to check your status.
